Requirements:

· Current California State NP license (for Nurse Practitioners)

· Current Nurse Practitioner Furnishing Number (for Nurse Practitioners) 

· Nurse Practitioner furnishing number of NPs (for Nurse Practitioners) 

· Current California State PA License (for Physician Assistants)

· CAPA Controlled Substance Certification (for Physician Assistants)

· Active DEA registration

Preferred:

· 6 months of related clinical experience
